About this calculator
The Rapid Shallow Breathing Index (RSBI), introduced by Yang and Tobin in a landmark 1991 study, is calculated as respiratory rate (breaths/min) ÷ tidal volume (in litres) during a brief trial of unassisted breathing. An RSBI under 105 is the widely used threshold suggesting a patient is likely to tolerate extubation, while higher values flag a fast, shallow breathing pattern associated with weaning failure.
Intensivists and respiratory therapists calculate RSBI at the bedside as one of the most reliable, simplest predictors available before attempting to wean a patient off mechanical ventilation — it doesn't replace clinical judgment, but it's cited in ventilator-weaning protocols worldwide as a fast, objective data point alongside oxygenation and hemodynamic status.
